- Once you click the above link you will be taken to the Tournament Center and you must create a username & password
- Complete Guest Player/Player Loan Form
- After completing all forms, print confirmation. This is what you show to the tournament

- 2011/2012 Tournament Roster Form
- To complete the online Tournament Roster Form, you will need to create a team account with Gotsoccer if you have not yet done so. Once you create your account, you can add each player manually to your tournament roster. When you have entered each player, you will submit your roster. Each day, an IYSA representative generates all tournament rosters that have been submitted. After your roster has been generated, you can log back into your Gotsoccer account and your tournament roster will be ready to print. If you need a stamp of approval on your tournament roster, email it to cep@illinoisyouthsoccer.org and the approved roster will be emailed back to you.

- TAKES 48 HOURS TO PROCESS AFTER ALL PAPERWORK IS RECEIVED. No passes processed on Fridays.
- Paperwork will NOT be processed until ALL of the following is received
- Copy of birth certificates of all players
- Medical Release Forms for all players
- Photos have been uploaded for all players and coaches (headshots)
- Coaches have submitted a background search
- Coaches need to provide a copy of an IYSA/US Soccer Coaching Education License appropriate to the team age group (ex: Youth Modules, E,D. ect)
- 2011/2012 Individual Tournament Player Pass(this is for a player who is not currently registered with a competitive team that would like to guest play with a competitive team for tournament play only)
- If you do not have a GotSoccer account you must create a new account in order to process the players pass
- TAKES 48 HOURS TO PROCESS AFTER ALL PAPERWORK IS RECEIVED. No passes processed on Fridays
