Coaching Conflict Form

This form must be completed if a coach is coaching multiple teams competing in the State Cup, Presidents Cup or Illinois Cup this Spring. Based on the information provided IYSA will build the match schedule to avoid as many potential coaching conflicts as possible. Please note that this does NOT guarantee a coach will not have a conflict, but we will try to avoid as many as possible.

THIS INFORMATION MUST BE SUBMITTED NO LATER THAN 3PM ON MARCH 14.

Coaches that do NOT submit this information will not be considered when building the event schedules.
