Incorporation Information
- To incorporate your organization, please go to http://business.illinois.gov/registration.cfm or go to your nearest Secretary of State’s office.
Insurance
- 2025/2026 Insurance Certificate Request Form (for facilities for coverage 9/1/25-8/31/26 – please allow 48 hours for processing and up to 7 days for certificates requiring special wording)
- 2025/2026 Insurance Claim Forms – for injuries sustained 9/1/25-8/31/26 (Must complete and submit all of the following below to mk@illinoisyouthsoccer.org)
- Incident Report – to be completed by league administrator to document an incident/injury in case of a lawsuit
- IYSA Medical Release & Waiver (12/1/24) – required for every participant
- Coach keeps original in case of an injury
- League keeps a copy on file to submit with insurance claim
- League retains a copy until the participant has reached 21 years of age
- Since the waivers contain private information, store them in a safe locked box
