State Cup, Presidents Cup & Illinois Cup Group Play Game Change & Home Team Host Submission Form

This form must be submitted no later than the following dates to be approved

FOR HOME TEAM HOST GROUP PLAY GAMES: Information must be submitted no later than 3pm on April 6 and/or NO LATER than 5 days prior to the to the agreed upon date, whichever comes first
FOR MOVING GROUP PLAY GAMES AWAY FROM ROCKFORD: Information must be submitted no later than 3pm on April 6 and/or NO LATER than 5 days prior to the to the agreed upon date, whichever comes first

Failure to submit all games designated as Home Team host by April 6 at 3pm will result in a fine to both teams.

Game changes submitted less than 5 days prior may be declined.

Teams can agree with their opponent to move their group play games scheduled in Rockford on May 16-17 to an EARLIER date and alternative location.

BOTH teams must be in agreement. If teams do not agree the match is played in Rockford as scheduled by IYSA on May 15-17
Moved games CAN NOT be played on May 15-17. If your teams want to play May 15-17 you must play in Rockford
All agreed upon changed games must be submitted by April 6 at 3pm and/or no later than 5 days earlier to the agreed upon play date
IYSA will send all referees for moved games
