Tournament Forms

THE IYSA OFFICE WILL BE CLOSED ON FRIDAY, MAY 24. ALL TOURNAMENT PASSES AND TOURNAMENT TEAM ROSTERS MUST BE SUBMITTED NO LATER THAN 12PM WEDNESDAY IN ORDER FOR THEM TO BE PROCESSED FOR THE MEMORIAL DAY WEEKEND.

• 2012/2013 Tournament Roster

To complete the online Tournament Roster Form, you will need to create a team account with Gotsoccer if you have not yet done so. Once you create your account, you can add each player manually to your tournament roster. When you have entered each player, you will submit your roster.

After you have submitted your online Tournament Roster, send an email to mk@illinoisyouthsoccer.org. Be sure to include the Team Name, Team Gender, Team Age Group, and the name of the coach and your approved Tournament Roster will be emailed to you.  If you have any questions, please contact Meaghan Kinser at mk@illinoisyouthsoccer.org.

• 2012/2013 Tournament Team Procedures  

These procedures MUST be followed for an Illinois team that is not currently registered for IYSA competitive play that only wants to play in tournaments. Read all details from the above link.  The fee is $15 per player and per coach.

TAKES UP TO 1 WEEK TO PROCESS AFTER ALL PAPERWORK IS RECEIVED. No passes will be processed on Fridays.

Paperwork will NOT be processed until ALL of the following is received
1.) Copy of birth certificates of all players. DO NOT UPLOAD TO GOT SOCCER. Birth certificate MUST be emailed to mk@illinoisyouthsoccer.org or faxed to 847-290-1576 
2.) Medical Release Forms for all players.  DO NOT UPLOAD TO GOT SOCCER. Medical releases MUST be emailed to mk@illinoisyouthsoccer.org or faxed to 847-290-1576 
3.) Photos have been uploaded for all players and coaches (headshots)
4.) Coaches have submitted a background search at http://www.illinoisyouthsoccer.org/BackgroundCheck/index_E.html
5.) Coaches need to provide a copy of an IYSA/US Soccer Coaching Education License appropriate to the team age group (ex: Youth Modules, E,D. etc)
 

• 2012/2013 Individual Tournament Player Pass

The Tournament Pass is for a player who is not currently registered with an IYSA competitive team to guest play in a tournament with an IYSA competitive team. If a player that is currently registered with an IYSA competitive team obtains a Tournament Pass, that player’s Tournament Pass will be considered void and any and all benefits derived from the use of the Tournament Pass including but not limited to insurance coverage are void. This pass is for tournaments only and not valid for league play.  The fee is $15 per player pass.

If you do not have a GotSoccer account you must create a new account in order to process the players pass.
