If a coach is coaching multiple teams in a Spring 2026 Cup (State Cup, Presidents Cup or Illinois Cup) the following form must be completed and submitted no later than Friday, March 14 at 12pm.
This form will be used to create a schedule to avoid, when possible, conflicts for coaches as we build the final Cup schedules.
Please note that submission of this form does NOT guarantee that all conflicts can be avoided.
