24/25 Insurance Certificate Request Form (for facilities for coverage 9/1/24-8/31/25) – please allow 48 hours for processing and up to 7 days for certificates requiring special wording
24/25 Insurance Claim Forms – for injuries sustained 9/1/24-8/31/25 (Must complete and submit all of the following below to mk@illinoisyouthsoccer.org
- 24/25 Claim Form
- IYSA Medical Release & Liability Waiver (12/1/24)
- Itemized Medical Bills
- Primary Insurance Explanation of Benefits
23/24 Insurance Claim Forms – for injuries sustained 9/1/23-8/31/24 (Must complete and submit all of the following below to mk@illinoisyouthsoccer.org
- 23/24 Claim Form
- IYSA Medical Release & Liability Waiver (12/1/24)
- Itemized Medical Bills
- Primary Insurance Explanation of Benefits
Incident Report – to be completed by league administrator to document an incident/injury in case of a lawsuit
IYSA Medical Release & Waiver (12/1/24) – required for every participant
- Coach keeps original in case of an injury
- League keeps a copy on file to submit with insurance claim
- League retains a copy until the participant has reached 21 years of age
- Since the waivers contain private information, store them in a safe locked box
